Background Checks
All adults working with our students must submit a background check application. Background checks are valid for two years.
People already approved in our system will receive automatic reminders:
- 30 days before their approval expires
- Another reminder, 15 days before expiration
People new to our system will still need to apply to begin the process.
How to Apply
To apply, follow these steps:
- Download and Sign the Policy Form

Download and sign the Lake Oswego School Board Policy Acknowledgement Form (best done from a desktop computer)

- Complete the Background Check Application

Fill out the Background Check Application for LOSD.

Upload the signed Policy Acknowledgement Form when prompted.

- Wait for Approval

Allow up to three weeks for approval.

You will receive an email notification once your application is approved.

The school will be informed of your approval status.

Required Information
To complete your application, you will need:

Email Address: For notification purposes.

Government-Issued ID Information: Ensure your full legal name matches.

Social Security Number: Required for the background check.

Signed Policy Acknowledgement Form: An unsigned form will result in automatic denial.
Note: Accommodations are made for alternative identifications, please contact Human Resource office for assistance.

Background Check: The LOSD Human Resources department will use the provided information for a background check.

Processing Time: Approval can take up to three weeks.
