Background Checks

All adults working with our students must submit a background check application. Background checks are valid for two years.

People already approved in our system will receive automatic reminders:

  • 30 days before their approval expires
  • Another reminder, 15 days before expiration

People new to our system will still need to apply to begin the process.

How to Apply

To apply, follow these steps:

  1. Download and Sign the Policy Form
  2. Complete the Background Check Application
  3. Wait for Approval
    • Allow up to three weeks for approval.

    • You will receive an email notification once your application is approved.

    • The school will be informed of your approval status.

Required Information

To complete your application, you will need:

  1. Email Address: For notification purposes.

  2. Government-Issued ID Information: Ensure your full legal name matches.

  3. Social Security Number: Required for the background check.

  4. Signed Policy Acknowledgement Form: An unsigned form will result in automatic denial.

Note: Accommodations are made for alternative identifications, please contact Human Resource office for assistance.

Additional Information

  • Background Check: The LOSD Human Resources department will use the provided information for a background check.

  • Processing Time: Approval can take up to three weeks.

  • Accommodations: If you need assistance with the background check, please email the LOSD HR department.
