Technology Consent and Device Protection
Complete your student’s required annual technology consent form and consider enrolling in the optional Device Damage and Loss Protection Program. Please follow the instructions below to get started.
- Sign the Required Technology Consent and Acceptable Use form for each student in your care, found in ParentSquare.
- This is required annually for every student and ensures safe and responsible use of district resources, devices, and software.
- You must be signed in to your ParentSquare account to complete the form.

- For $40 per student ($120 family maximum), this program reduces the financial burden for accidental repairs or replacement of school-issued devices. Without coverage, charges can be as high as $450.

Technology Consent
Parents must sign a technology consent form to ensure the responsible use of district-provided devices, software, and online tools for educational purposes. This consent helps the school comply with privacy laws like FERPA, COPPA, and CIPA, which protect student information and ensure safe internet use. Additionally, it allows parents to opt into programs such as the Device Damage and Loss Protection Program, which can safeguard against potential costs related to damaged or lost devices.
LOSD Resources
Lake Oswego School District (LOSD) provides a vast array of technological resources to all students at all grade levels in keeping with current and established educational practices. These resources include (but may not be limited to):
- Tablets, laptop computers, and other devices
- Google Suite for Education (Gmail, Google Classroom, etc.)
- Online instructional and educational applications
- Digital textbooks
- Video conferencing for remote learning
LOSD strives to ensure that the systems and applications it uses are compliant with the Family Educational Rights and Privacy Act (FERPA), the Children's Internet Protection Act (CIPA), the Children's Online Privacy Protection Act (COPPA), and the Health Insurance Portability and Accountability Act of 1996 (HIPAA), if applicable.
The Lake Oswego School Board has adopted a number of policies that address technology use. These include IIBGA - Electronic Communications System and JFCEB - Personal Electronic Devices and Social Media. In addition to these policies, the District expects parents and students to know and understand the following expectations regarding the use of technology. By acknowledging this document, parents confirm that they understand their student(s) will have access to LOSD technology resources, and that they have reviewed these expectations with each of their students attending any LOSD school.
Software Consent
Google Apps for Education Policy
Lake Oswego School District utilizes Google Apps for Education. In order to access these resources, your student will be assigned a full Lake Oswego School District 7J Google Apps for Education account. This means your child will receive an email account, access to Google Docs, Calendar, and Sites. While student privacy remains a top priority in the Lake Oswego School District, students may be able to selectively release educational records by changing the security setting profiles of his/her account. Access to and use of Google Apps for Education and to district technology is considered a privilege accorded at the discretion of the Lake Oswego School District. The district maintains the right to immediately withdraw the access and use of Google Apps for Education when there is reason to believe violations of law or district policies have occurred. In such cases, the alleged violation will be referred to the principal for further investigation and account restoration, suspension or termination. As a tenant of the Agreement with the State of Oregon, the state reserves the right to immediately suspend any user account in question of appropriate use. Pending review, a user account may be terminated as part of such action.
The District uses other software, applications and extensions for educational purposes.
Zoom and Google Meet Permission
For remote sessions, the Lake Oswego School District utilizes Zoom and/or Google Meet. These real-time video conferencing tools will allow teachers to engage in short class meetings to build connections with students during Remote Learning. In addition, Lake Oswego teachers may use live video conferencing for check-ins and as a way to provide feedback to students. This tool may also be used with related services to support individual student goals. During the use of live video conferencing, students are expected to respect the privacy of others. Students are not allowed to take pictures or screenshots, or record any sessions. In addition, students should use proper online etiquette, including muting themselves when they are not talking and taking turns, so that everyone can participate in the discussion. When the session is over, students must close out the app once the teacher has ended the session. Most importantly, students are expected to be kind and respectful during the live video conferencing session.
Software/Apps
Lake Oswego School District uses various 3rd party software, apps, extensions, and more.
By providing consent in the Payment Portal, I acknowledge that I have read and understand the Software Consent policies. I understand that my child will be assigned a full Google Apps for Education account, and agree to the terms and conditions of the Google Apps for Education Policy. I also give permission for my child to receive and/or transmit audio/video images via Video conferencing software; and I agree to the use of other District-approved software, applications and extensions.
Acceptable Use Policies
Student Internet Acceptable Use Policies
Elementary Students (Grades K–5)
- The student understands that their use of technology at school and for school should be limited to school-related activities in support of their education.
- The student understands that they should behave appropriately and safely while using technology, including protecting their personal information.
- The student understands that school-related technology use is not private and could be subject to review at any time.
- The student understands that they should use technology in ways that their teachers and family are aware of and are in support of.
- If the student should encounter anything strange or unexpected while using technology, they will seek the help and guidance of a teacher.
- The student will do their best to learn what it means to be a good digital citizen and strive to become a good digital citizen, both at school and outside of school.
- The student respects that the adults who work at school can direct their use of personally owned technology while at school (including on buses) and that this may include being asked to leave it at home or put it away.
Secondary Students (Grades 6 and higher)
- All use of district technology should be in support of the student’s education. The student will not personalize or change settings of any device that could be used by others. The student will try not to damage or permanently mark any school-owned technology. The student understands that they are responsible to repair/replace any damage done to school-owned technology.
- The student will sign up for online services using their district account only in pursuit of educational objectives in consultation with a teacher. The student will not use district-provided email or information in order to sign up for non-school-related services.
- The student will try their best to learn what it means to be a good digital citizen and strive to become a good digital citizen, both at school and outside of school.
- The student knows that vandalism of any kind or intentional misuse of district electronic systems (including attempts to bypass the district’s content filtering system) is strictly prohibited and may be met with disciplinary action.
- The student recognizes that they have off-site access to many technological tools and resources and that use of these tools is subject to the same expectations as for appropriate use at school.
- The student respects that the adults who work at school can direct their use of personally owned technology while at school (including on buses) and that this may include being asked to leave it at home or put it away. The student will always check with teachers or administration regarding the specific rules before bringing technology to school or school buses. If allowed at school, the student knows that these tools are subject to district use guidelines.
- Whenever possible, the student will use district-provided systems to accomplish educational objectives. If a system asks for personal information such as full name, address, ID number, or birthdate, the student will alert a teacher and proceed only after adult guidance.
- Use of a district-provided account of any kind should be limited to the student. The student will never access any other students’ accounts. The student will not share their passwords with other students. Any time the student suspects that a password to a district-related system has been intentionally or accidentally communicated, they will change it.
- The student understands that they have the right and obligation to protect their files and other resources. Among other reasonable and appropriate measures, this includes periodic changing of passwords, properly securing devices, and exiting/logging out of data systems when not in active use. The student will not leave technology tools logged in when they are done using them.
- When using district technology and systems, the student understands that they are expected to use appropriate tone and content for the educational environment. The student will behave online realizing that anything they do could potentially be viewed by employees of the school district, even perhaps without their knowledge.
Parent/Guardian Acceptable Use Policy Acknowledgement
By providing consent, I acknowledge that as the parent or guardian of this student, I have read the district’s Acceptable Use Policy (AUP) and this contract. I understand that controversial material is available on the Internet and I permit my child to use the Internet despite this potential availability. By doing so, I agree to enforce acceptable use when my child is off district property. I will not hold the district responsible for materials my child acquires on the network. My child uses the Internet at my child’s own risk and at my own risk. I hereby release the district from any claim arising from my child’s use of the Internet. I agree to report any misuse of the Internet resources to a district administrator. I understand that my child’s violation of the district’s computer AUP may result in the loss of Internet access and/or my child’s user account; may result in other disciplinary action; and may constitute a criminal offense. I hereby give my permission for my child to access the Internet/network and I give permission to the district to issue an account for my child. I certify that the information contained on this application is correct. I also give permission for my child to use Google Apps for Education and other third-party services. The law allows the District to consent to the collection of personal information on behalf of its students, thereby eliminating the need for individual parental consent given directly to the service operator. A complete list of these services offered by the District for educational purposes is available from the District office. This form will constitute consent for our schools to provide your child’s personal identifying information to the service operators on file with the District office.
Device Damage and Loss Protection Program
- This program is optional but highly recommended.
- Coverage begins immediately upon payment before the deadline.
- Fee waivers are available for qualifying families.
Enrollment Deadlines:
- The 2024-2025 deadline to enroll in the Program and submit a payment or fee waiver is 10/19/24. Insurance is not in effect until payment or fee waiver is received.
- Students who are new to and enroll in the District mid-year will have 30 days from their start date to enroll and submit the payment or fee waiver.
LOSD Device Damage and Loss Protection Program
LOSD students who are assigned a District technology device (Chromebook or iPad) are responsible for all damages or loss, whether intentional or accidental, and regardless of whether damage or loss occurs on or off of District Property.
Each student in Grades K-2 will be assigned an iPad. Each student in Grades 3-12 will be assigned a Chromebook. Only secondary school students may be given the option of using a personal device in place of a Chromebook, and the Program does not offer protection to these student-owned, personal devices.
Enrollment in the Device Damage and Loss Protection Program is optional and it provides an inexpensive solution for parents to reduce the financial burden if an accident, loss, or theft occurs. Uninsured costs can be as high as $450. Parents/guardians may choose to not participate in the LOSD Device Damage and Loss Protection Program, but will be fully responsible for any associated cost for damage, loss, or theft, regardless of whether incidents occur at home or at school.
The 2024-2025 program cost is $40 ($120 Family Max). Two incidents of accidental damage or loss are covered per year. If you cannot afford this fee, a District fee waiver is available to qualifying families.
Below are examples of the cost of replacement/repair for an insured versus a non-insured device. Repair bills will reflect the actual cost of each repair.
Please note that lost or damaged chargers are not covered under the program.

2024-2025 Annual Program Cost:
- $40 per student. Maximum $120 program enrollment fees per family. A fee waiver form is available for qualifying low-income families on the District’s website. Please submit the form to your student’s school and obtain Administrator approval before enrolling online.
- The LOSD Device Damage and Loss Protection Program will cover one school year (August to August) and is non-refundable. New students receiving a device after February 1 of each calendar year may request prorated coverage.
What are the repair and replacement charges for devices and accessories?
- For those who do not participate in the Program, or who have exceeded two repairs per year, charges to families are based on the actual cost of the repair or replacement. Repair costs can range from $15 to $219 for each part needing repair. Device replacement costs range from $350-$450. If the repair cost equals or exceeds the cost of replacement, you will be billed for a replacement.
What is NOT covered by the program?
- Lost or damaged charging cords or adapters.
- Intentional damage, misuse, abuse, willful or negligent damage caused by the student or any household members. Determinations will be made by District Administrators/CNS.
General Terms:
- Two claims per year due to accidental loss or damage.
- Any damage or loss must be reported to staff right away.
- All repair, loss, or incident fees are per device. Family maximum does not apply.
- If a lost or stolen Chromebook/iPad is later recovered, the repair/replacement fee or co-pay will be refunded.
- If a student leaves the District but does not return the Chromebook/iPad, they will be fined for the full replacement cost, and standard rules for the restriction of student records and transcripts will apply.
- If you do not enroll in the LOSD Device Damage and Loss Protection Program, you will be financially responsible for the full cost of the repair or replacement of the Chromebook/iPad/Case, regardless of the incident location.
