Home Address

  • New LOSD Parents: Enter your own home address. You will have the opportunity to enter addresses for other parent/guardians later in the process.
  • Current or Returning LOSD Parents: select "Check here if your address has changed" if applicable; otherwise, just press Save And Continue. You will be asked to provide proof of residency to your child's school if your address has changed.

Families with an approved interdistrict transfer or tuition agreement must enter the enrolling school's address, then follow up with the school to confirm your actual home address.

Mailing Address

This screen is for your own mailing address. Check "Mail address is the same as home address" if applicable. If your mailing address differs from your home address, fill in the fields.
