• Lake Oswego School District Device Damage and Loss Protection Program

    Lake Oswego School District (LOSD) students assigned a district technology device (Chromebook or iPad) are responsible for all damages or loss, whether intentional or accidental. Responsibility for repair caused by a malfunction or maintenance issue will be determined by the Lake Oswego School District’s Computer Network Services (CNS) Department.

    Enrollment Information

    The Device Damage and Loss Protection Program is optional and provides an inexpensive solution for parents to reduce the financial burden if an accident, loss, or theft occurs.

    Annual Price: $20 per student. Maximum $60 program enrollment fees per family. Fee waiver is available.

    Parents/guardians may decline to participate in the LOSD Device Damage and Loss Protection Program, but will be fully responsible for any associated cost for damage, loss, or theft.  The LOSD Device Damage and Loss Protection Program will cover one school year (August to August) and is non-refundable.  Students receiving a device after February 1 of each calendar year will be eligible for prorated coverage.

    Prior to receiving a device, the following must be completed:

    1. Complete the LOSD Student/Parental Consent & Technology Form to accept or decline participation in the Device Damage and Loss Protection Program.
    2. If coverage is selected, pay the $20 annual program fee, per student, in full. $60 maximum fee per family.     Payment is due by October 1, 2020, to insure your child's device.

     (1st and 2nd incident fees will be waived if the device is stolen and a police report is filed)

    Cracked/damaged screen or other Chromebook/iPad damage due to accident

    (example: headphone jack or charger broken off inside device, damaged screen, etc)

    Repair charges are based on the average of actual professional repair service costs by category/device as listed in Chart 1.

    1st incident: $0

    3rd incident: full cost of repair (see Chart 1) 

    (Accident must be reported within one week of incident)

    (example: will not charge or turn on, faulty speaker, defective microphone, charging accessories, etc)

    NOT COVERED BY DEVICE DAMAGE AND LOSS PROTECTION PROGRAM:

    • Charging Cords
    • Charging Adapters
    • Intentional Damage (as determined by district administrators/CNS)

    GENERAL TERMS:

    • Two incidents total per device, in any combination (damage/stolen), are covered per year.
    • All repair, loss, or incident fees are per device. Family maximum does not apply.
    • If the lost or stolen Chromebook/iPad is later recovered in working condition, the repair/replacement fee will be refunded.
    • If a student leaves the district, but does not return the Chromebook/iPad, they will be fined for the full replacement cost, and standard rules for the restriction of student records and transcripts will apply. 
    • If you choose not to enroll in the LOSD Device Damage and Loss Protection Program, you will be financially responsible for the full cost for the repair or replacement of the Chromebook/iPad/Case.

     You will accept or decline to enroll in the Damage and Loss Protection Program when you fill out the LOSD Student/Parental Consent & Technology Form.
